Make the avg number of trails a passing criteria (#398)

Units should have a miniumum avg number of trials before they are
considered as passing. Make the default < 2 to avoid blocking too
eagerly.

The `min_fraction` field in the passing options and the filtering of
easy and mastered exercises had to be relaxed to compensate for the
additional requirement.
