add: publish GitHub releases from a tag-driven GitHub Actions workflow after required checks succeed
add: extract release notes with a reusable script for local and CI release flows
change: make tools/publish_release.sh push only the branch and version tag while CI creates the release
change: document the new automated release publication flow in the README
