fix(ci): fix claude workflow permissions and null body guards (#1734)

* fix(ci): fix claude workflow permissions and null body guards

- claude.yml: coalesce null bodies with || '' before contains() to
  prevent expression errors on empty review/issue bodies
- claude.yml: grant write permissions so the action can post comments
  (contents/pull-requests/issues: write)
- on-pr.yml, on-main.yml: narrow permissions from write-all to
  contents: read (orchestrators only call reusable workflows)
