fix $adhoc macro being replaced by templateSrv when adhoc variable exists

Protect $adhoc plugin macro from Grafana's templateSrv.replace() using a
placeholder strategy. When an adhoc variable named "adhoc" is configured,
templateSrv treats $adhoc as a template variable and replaces it with a
quoted string literal, breaking the backend macro processing.

Fixes regression of issue #422.
