[TASK] Rename variables etc. in `CssConcatenator` (#1596)

The new names more accurately reflect the nomenclature of the formal
specification, and match the naming used in the recently-extracted classes
that are used, thus making the code easier to understand.

Changed are
- DocBlock comments
  - The spec uses the term 'rule set' whereas MDN uses 'ruleset';
    'ruleset' is easier to read and understand when used in a sentence;
- parameter names;
- local variable names;
- private method names.

Also add description for the internal property, which is a list of lists,
to aid understanding.
