vinyl: rename vy_lsm::mem_format to format

vy_lsm::mem_format is the format used for allocating tuples. It's used
for statements stored in the memory level in all indexes. For the
primary index it's also used for storing REPLACE, INSERT, and UPSERT
statements on disk (DELETEs are stored in the key format). It's obvious
that the name is a misnomer. Let's rename it to vy_lsm::format.
