remove local coveralls push and coveralls dep

- package.json: remove npm run coverage, which would do a local push to
  coveralls.io. since npm run test already reports text coverage this is
  unnecessary and CI still pushes.
- remove coveralls package dependency which is 5 years old and has tons
  of vulnerable dependencies.

- npm audit fix --force
- bump mocha to newer (but still vulnerable) version.
