docs: rewrite usage.md

- Modernize structure and tone to match the new README style
- Fix incorrect middleware export syntax (was arrow function, now regular function)
- Replace wall-of-text descriptions with tables for response builder and request params
- Add --always-fake-optionals and --build-cache to CLI reference
- Consolidate context/REPL/loadContext content into logical sections
- Remove stale "coming soon" feature list and No Cap Recap summary
- Fix minor code errors (missing = in GET signature, stray backtick in response chain)
