[sidecar] add limits (#420)

#### Description

Reject notification requests that exceed MaxTxIDsPerRequest with a
RejectedTxIds response containing the offending txIDs and a reason.

Track active subscription count. When a new request would exceed
maxActiveTxIDs, reject full/partial with a rejection response.

#### Related issues

 - resolves #405
