feat: pluggable rate limit store with secure in-memory backend (#51)

- Add RateLimitStore interface for custom backends (Redis, etc.)
- Add secure in-memory store with lazy expiration and LRU eviction
- Fix DefaultKeyExtractor to strip ports using net.SplitHostPort
- Add context support to store interface
- Move store implementation to middleware/rate_limit_store.go
