Tighten mutation regex and add tests for false positives

Use a lookbehind to exclude word chars before "mutation" and only
match uppercase letters after it (plus { and (), so filenames like
update_campaigns_mutation_spec.rb and commit messages mentioning
mutations no longer trigger the ban-write-operations rule.

Add tests for rm, git rm, and git commit with "mutation" in
filenames and messages.
