fix: reduce cyclomatic complexity in TestAzureVMProvisionMode

Extract optionsTestCase type and runOptionsTest helper to reduce the
cyclomatic complexity of TestAzureVMProvisionMode from 16 to within
the threshold. Remove t.Parallel() since subtests modify environment
variables via saveAndClearEnv.
