feat: add azurevm provision mode with relaxed validation

Add new "azurevm" provision mode for generic Azure VM provisioning
that doesn't require AKS-specific configuration (cluster-endpoint,
kubelet-bootstrap-token, ssh-public-key, cluster-name). Only
vnet-subnet-id and node-resource-group are required. Includes
IsAzureVMMode() helper and comprehensive unit tests.
