perf: [alb] optimize fanout mechanisms + fix race and edge cases

Optimize ALB fanout mechanisms, fix a write-after-return race in FR, and add comprehensive test coverage.

- fix: FR write-after-return race on context cancellation
- fix: missing nil guards in FR, NLM, and UR
- perf: add CloneWithoutResources to avoid wasted clone in FR/NLM/TSM fanout
- perf: remove mutex from NLM hot path, use per-goroutine slots
- test: comprehensive coverage for UR, FR (FGR + race detector), NLM, RR
