Fix bugs, add query builder features, tests, and CI

Bug fixes:
- Fix "PostreSQL" typo in engine.go
- Fix "contnue" typo in escape.go
- Fix wrong error message in FetchGrouped (said "FetchMapped")
- Fix Set.Unset() doing nothing (slices.Delete range was empty)
- Fix InsertIgnore for PostgreSQL (ON CONFLICT DO NOTHING)
- Fix Replace for PostgreSQL (ON CONFLICT DO UPDATE)
- Fix zero time format for PostgreSQL (use 0001-01-01 epoch)

New features:
- Add GroupByFields() builder method
- Add Having() builder method
- Add SetDistinct() builder method
- Add Join/LeftJoin/InnerJoin/RightJoin builder methods

Tests & CI:
- Add comprehensive unit tests (80+ test cases)
- Add GitHub Actions workflow with Go 1.23/1.24 matrix
- Add Coveralls coverage integration
- Add Build Status, Coverage, and Go Report Card badges to README
- Document new features in README
