Add PR template and Claude Code rule for PR creation (#4032)

* Add PR template and Claude Code rule for PR creation

Adds a GitHub pull request template with sections for summary, type of
change, test plan, changes table, user-facing changes, and reviewer
notes. Also adds a Claude Code rule to ensure agents follow the template
when creating PRs.

Use explicit MUST/REQUIRED language in both the PR template comments and
the Claude Code rule so contributors and agents cannot skip the why,
