docs: fix remaining inconsistencies in design.md

- serverInfo: rename stateCounter to trustStateCounter, add
  testInstance and trustStateHash fields
- accounts: rename independentPaths to pathCount, add depth field
- trustEdges: add missing invalidated field
- tasks: use notBefore (camelCase), use real task names in example
- Add note about _loading intermediate collections
- Add TODO in Task.java to rename "not-before" to "notBefore"
