add Coveralls coverage reporting, fix old format offset computation

- Add Coveralls badge to README
- Add coverage profiling and Coveralls upload to CI workflow
- Replace magic-scanning in Open() with deterministic offset computation
  matching glibc's ALIGN_CACHE(sizeof(cache_file) + nlibs * sizeof(file_entry))
