fix: node 18 compat for remark, lint-staged, and windows test runner

- Downgrade remark-cli from 12.x to 11.x (12.x + remark-preset-github
  causes ERR_REQUIRE_ESM on Node 18 via remark-gfm ESM/CJS conflict)
- Remove pnpm.overrides for remark-gfm (no longer needed with 11.x)
- Downgrade lint-staged from 16.x to 15.x (string-width 8.x uses /v
  regex flag unsupported on Node 18)
- Replace node --test test/*.js with node --test (Windows glob fix)
- Add coverallsapp/github-action@v2 and --reporter=lcov to c8 scripts
- Fix CI/Coveralls/npm badge URLs in README
