Add docs, rewrite README, update tests

Add a full docs/ directory (getting-started, configuration, usage, commands, advanced, testing, index) and substantially rewrite README with streamlined examples, installation notes and links. Add a VSCode PHP version setting and rotate the jwt.signer value in .env. Update phpunit.xml.dist and adjust tests/Commands/JWTGenerateKeyActualTest.php to match the changes. These changes expand documentation, clarify usage/CLI commands, and refresh test/config wiring; note that .env contains a new signing key and should not be committed.
