feat: add cwd parameter to MCP tools for reliable index root detection

When Claude passes path="/project/src" and no parent index exists,
findEffectiveRoot walks up but finds nothing, indexing only src/ and
missing the rest of the project. The new optional cwd parameter lets
Claude pass its working directory as the authoritative index root.

- Add Cwd field to SemanticSearchInput and IndexStatusInput
- Update validateSearchInput with cwd/path defaulting and validation
- Add preferredRoot parameter to getOrCreate, bypassing findEffectiveRoot
- Update SessionStart hook to instruct Claude to always pass cwd
- Add pathCrossesSkipDir to prevent parent reuse across SkipDirs
- Add tests for preferredRoot, cwd/path validation, and SkipDir logic
