refactor: Build custom LNbits image with baked-in patch, address PR review

- Add Dockerfile.lnbits pinned to exact image digest (v1.5.0)
- Bake lnbits#3817 patch at build time via patches/ script
- Remove brittle runtime find+patch from entrypoint
- Use CLN CA cert for TLS verification in REST readiness probe
- Narrow LightningPaymentError to 400/402/409 only
