docs(grcc): add GPT-5.3-Codex commentary

- add `@file` headers and module `@defgroup`/`@ingroup` organization
- expand `@param` docs for pointer/array-heavy APIs with in/out semantics
- document ownership/lifetime contracts for callbacks and Assign inputs
- add invariants/preconditions/postconditions for key assignment flow
- refine ambiguous comments to evidence-based wording and fix minor typos
