feat(bdd): Implement Scenario Outline expansion (REQ-BDD-005)

- Support multiple Examples tables per Scenario Outline
- Add expand_outline() function for expanding outlines to scenarios
- Substitute placeholders in step text using Examples table values
- Preserve tags inheritance from outline and examples to expanded scenarios
- Track outline line number and example row index for traceability
- Add --expand-outlines flag to parse-feature CLI command
- Support Examples table descriptions and tagged examples
- All 12 outline tests pass
