* fix: protocol audit — required tags, naming consistency, hardcoded values

1. refs: make target tag required (ref without commit SHA is meaningless)
2. patches: add from/to tags to statusChange (parity with issues protocol)
3. patches: rename tipCommit → headCommit in revisionBundle schema
4. releases: read commitSha tag for target_commitish instead of hardcoded 'main'
5. credential-cache: mkdirSync parent dir before writeFileSync

Closes #127

* fix: update tests for tipCommit → headCommit rename in revisionBundle

The protocol spec test and E2E collaboration test still referenced the
old tipCommit tag name after the revisionBundle rename in patches.ts.
Update both to use headCommit.

* chore: retrigger CI (Coveralls rejected stale build ID)
