Fix pg_up reporting and keep /metrics available when DB is down

- Implement resilient startup with lazy connection pooling and timeouts
- Update /metrics to always return HTTP 200 during outages
- Improve pg_up metric accuracy and prevent it from being overwritten
- Omit database metrics when unreachable
- Refactor unit tests to prevent hangs and handle dynamic test database labels
- Update dependencies and address linting issues
- Update README and CHANGELOG
