Replace inline expandable forms with FAB + dialog pattern on Android (#595)

Replaces the inconsistent in-page expandable Card forms with a unified
FloatingActionButton (bottom-right "+") that opens an AlertDialog for
data entry. This matches the existing pattern used in HomeScreen and
provides a consistent UX across all screens.

Screens converted: EmergencyPlanScreen, FoodLogScreen, SportLogScreen,
MedicationLogScreen, MedicationListScreen, ActivityLogScreen,
DiaryScreen, SleepTrackingScreen.
