Fix Android habit categories: require category selection and allow inline creation (#591)

The category dropdown in AddHabitScreen and EditHabitsScreen allowed
selecting "No Category" and had no way to create new categories. This
removes the no-category option, adds validation that prevents saving
without a category, and adds a "New category" dialog for inline
creation. Also adds saveCategory() to AppViewModel supporting both
local (Room) and remote (API) storage modes.
