Fix habit evaluation card appearing off-screen on Android (#584)

Move the HabitEvaluationCard from a fixed position at the top of the
LazyColumn to inline directly below the selected habit item. Previously,
tapping a habit lower in the list would render the evaluation card
(with Complete/Remove buttons) near the top, out of view. Now it
appears right where the user is looking.
