Upgrade runtime to Elixir 1.17, OTP 26, Node 20

Phase 1 of the Phoenix/Elixir upgrade path — runtime only,
no Phoenix or LiveView version changes.

Changes:
- .tool-versions: Elixir 1.17.2 (OTP 26), Erlang 26.2.5, Node 20.18.0
- Dockerfile: Bookworm base images, remove OTP 23 SSL workaround
  for esbuild download, fix `as` -> `AS` Docker syntax
- CI: Use erlef/setup-elixir + actions/setup-node instead of Alpine
  container, update actions/checkout to v4
- mix.exs: Bump elixir requirement to ~> 1.14, remove deprecated
  :phoenix compiler (not needed since Elixir 1.14+)
- fly.toml: Migrate from legacy [[services]] to v2 [http_service]
