Fix Telegram notifications and markdown rendering (#5)

* Fix Telegram notifications and markdown rendering

Always send new messages instead of editing existing ones so users get
notifications on every tick. Have the LLM produce Telegram HTML directly
(<b>, <i>, <code>, <pre>) instead of Markdown, removing the fragile
_md_to_html conversion layer.

* Fix flaky Loki test double-counting across query types

The mock returned the same error response for all 4 Loki queries
(level + detected_level), causing lines to be counted twice.
Return error response only for the first query, empty for the rest.
