Update FHIR operation docs to organize sidebar and index file (#8484)

* Update operations index and sidebar navigation

⚠️ CRITICAL DEPENDENCY: This PR MUST be merged AFTER all other PRs (PRs 1, 3-10) have been merged into main.

- Reorganize operations index from DocCardList to categorized list
- Add all new operations to sidebar navigation
- Update markdown-to-mdx tooling

This PR references all new operation documentation files. If merged before those PRs, the build will fail with broken links.
