fix(pipeline): try DOI fallback even when Unpaywall returns non-PDF URLs

Unpaywall sometimes returns non-PDF URLs (e.g. doi.org redirects)
that silently fail the content-type check. Previously, the DOI-based
fallback was only tried when Unpaywall returned zero URLs.

Now fallback URLs are always appended after Unpaywall URLs (deduped),
so they act as a safety net when Unpaywall URLs return HTML instead
of a PDF.
