refactor: replace vtable indirection with typed function references

- AST nodes carry pipe.F, pipe.FF, monoid.Monoid, func(*Cell) bool directly
- Add App type as root of control-flow AST (replaces Program)
- Update ADR 0010 to reflect functions-first compilation model
- Update tests to use typed function references with it/v2
- Remove stale vtable comments from ast.go
