* docs(access): clarify per-patient CareTeam tenancy pattern and cardinality

- Use per-patient CareTeam names in Step 1 diagram (alice-smith, bob-jones)
  to signal that fine-grained tenants are valid and intended
- Remove shared-patient node; per-patient CareTeams are 1:1 with patients
- Add sentence to CareTeam bullet explaining the pattern works at any
  granularity, including one CareTeam per patient
- Add cardinality note in "multiple tenants" section: 100 patients →
  100 access entries is expected and supported

Clarify that each patient should have their own CareTeam resource, and
explain how clinicians accumulate access entries through ProjectMembership.
Add reference to provider organizations documentation.
