Add account menu for logged in patrons (PP-1943) (#96)

## Description

- Replaces the "Sign Out" header menu item with an "Account" menu that
includes a new copyable patron ID.
	- Moves the "Sign Out" button into the new menu.
- The "Sign In" item remains in the header, but is replaced by the
"Account" menu once the patron has authenticated.

## Motivation and Context

It is useful for users to be able to communicate their patron
authorization ID when seeking support.
