fix: simplify ralphex-update skill to skip do-nothing config files

All-commented files are functionally identical to missing files — embedded
defaults handle them at runtime. The skill no longer offers to auto-update
or install these files. Categories collapsed from 4 (A/B/C/D) to 3 (skip
do-nothing, skip unchanged, smart merge). Comparison algorithm clarified
to strip comment lines from both sides before diffing. Updated llms.txt
to reflect the simplified workflow.
