Add --dump-defaults flag and /ralphex-update skill (#109)

* feat: add --dump-defaults flag and /ralphex-update skill

Export raw embedded defaults for comparison with user's customized
config. Add Claude Code skill for smart semantic merging of updated
defaults into customized prompts/agents.

Related to #108

* fix: address code review findings

- fix isResetOnly to check DumpDefaults field, preventing --dump-defaults
  from being silently ignored when combined with --reset
- add /ralphex-update to README Claude Code Integration section (available
  commands table, installation text, manual installation links)
- add tests for handleEarlyFlags and isResetOnly functions
