feat(server): render markdown files as formatted HTML

Markdown files (.md, .markdown) are now rendered as formatted HTML instead
of raw source text. Uses goldmark with GFM extensions for tables, task lists,
strikethrough, autolinks, and fenced code blocks with chroma syntax highlighting.
Rendering is always enabled (not gated by --syntax-highlight flag).
