feat: add "Other" option for custom answers in plan creation questions

Related to #102

- append "Other (type your own answer)" to every QUESTION option list
- handle fzf exit code 1 (no match) by falling back to free-text input
- support custom answer in both fzf and numbered selection paths
- filter duplicate sentinel if model-generated options contain it
- add noFzf field to TerminalCollector for testable AskQuestion flow
- update README and CLAUDE.md to reflect new behavior
