feat: add --config-dir flag and RALPHEX_CONFIG_DIR env var (#100)

support custom config directory via --config-dir CLI flag or
RALPHEX_CONFIG_DIR environment variable, overriding the default
~/.config/ralphex/ location. empty configDir falls back to the
default. runReset accepts io.Reader/io.Writer for testability.
updates documentation across README, llms.txt, and CLAUDE.md.
