fix(create): guard nil provider for SSH provider mode (#643)

provider.Create() panics with nil pointer dereference when Spec.Provider
is ProviderSSH because the SSH case never assigns a provider. Skip the
Create() call when provider is nil (SSH mode uses pre-existing infra).

Audit finding #1 (CRITICAL).
