fix: strengthen review signal handling in prompts (#95)

- review prompts: require all agents to finish before evaluating findings
  or deciding on signals, preventing premature REVIEW_DONE emission
- codex/custom eval prompts: add explicit third state for when all findings
  are dismissed as invalid — no signal emitted, loop re-runs external tool
