* Add comprehensive OTEL telemetry documentation and migration guide

Update observability docs with complete metrics reference, span attributes,
configuration options, distributed tracing, and security considerations for
the OTEL telemetry improvements merged in issues #3399, #3764, #3765. Add a
new telemetry migration guide covering legacy-to-new attribute mapping and
the useLegacyAttributes dual-emission flag.

* Fix active connections legend and add OTEL semconv dashboard

Fix the Active Connections legend format in all three existing Grafana
dashboards to use the correct metric attributes (server, transport)
instead of the incorrect mcp_method and status labels.

Add a new dashboard for the OTEL MCP semantic convention metrics
(mcp.server.operation.duration) with panels for operation rate/duration
by method, tool call rate/duration by tool name, transport breakdown,
and error rate by type. Uses template variables for datasource and job
to work across CLI and K8s deployments.
