Refactor to Ports & Adapters with Command/Query DTOs (#10)

Align codebase with CLAUDE.md architecture standards:
- Fix domain: rename requiredAbility to required_ability, add audit to Project
- Define Port behaviours (OrganizationRepository, SimulationRepository, BoardRepository)
- Agents implement Port behaviours via @behaviour
- Usecase GenServers delegate to Agents instead of reimplementing logic
- Introduce Command/Query DTOs for Organization and Simulation use cases
- Remove placeholder hello/0 modules, fix credo pattern-matching consistency
- Apply mix format across all files
