fix: preserve markdown headers in prompt and agent files

stop stripping # comment lines from prompt/agent content when loading, as it destroys markdown headers (## Step 1, # Title, etc.). stripComments is now used only for emptiness detection (fallback to embedded defaults). add stripLeadingComments for frontmatter parsing in agent files with leading comments.
