Phase 3: peer discovery and automatic mesh formation

Nodes learn about other peers through PeerExchange messages and
automatically form direct connections, converging any topology into
a full mesh.

New: PeerExchange (msg_type=4) with PeerInfo, discovery table
(knownPeers persisted to bbolt), exchange loop (30s), discovery
loop (10s), exponential backoff, MaxPeers enforcement with early
reject, AdvertiseAddr config, bootstrap address dedup, node_id
validation (sha256 check) on merge.
