feat: fix CI check failure + add unit tests for checker engine and CLI

- Add 'pre-commit install' to axm-init.yml and scaffold template
- Add test_checker_engine.py (7 tests: engine run, formatters)
- Add test_cli_commands.py (10 tests: check, version, init with mocks)
- Update README.md and docs/index.md with correct check count (39)
- Coverage: checker.py 0→100%, cli.py 0→64%, overall 98%
