refactor: improve audit score to ~100/100

- Extract _check_pypi_availability and _print_init_result from cli.py:init (cc 13→5)
- Add tests for reserver subprocess paths (build/publish/reserve flow)
- Add tests for CLI JSON output, init failure, PyPI error edge cases
- Add tests for models validation, adapters errors, filesystem rollback
- Coverage 92% → 98% (348 tests, +25 new)
