feat: register all rules, cleanup legacy, update docs

- Register 20 rules in 8 categories (quality, security, dependencies,
  testing, architecture, practice, structure, tooling)
- Fix pip-audit JSON parsing (dict vs list output format)
- Rewrite all docs for 6-category scoring model (20/20/15/15/15/15)
- Add CLI examples and formatters to docs
- Update README with full feature set
- 142 tests pass, mypy clean, ruff clean
